Please advise on when to send an operative report, consults note or physicians note with the claim to the insurance to insure that the claims are not delayed for payment due to the insurance needing additional information.

Only send them when the insurance is asking, always let them go electronically. Sometimes they don't need them and no one wants their payments delayed.
 
Thank you for your input, so even if it is a surgery ( I will be billing for a Neurosurgeon) no operative report is sent to the insurance?

If it's a Commercial and/or Medicare - don't send until they request. If it's a Workman's Comp or Auto - The claim needs to be sent by paper with records 9 times out of 10.
