Wiki New vs Established Patient visit

Good afternoon everyone,

I have an insurance carrier denying a New Patient visit because a stress test was read by us 5 days prior to the E/M service. This patient is new to us.

It was my understanding diagnostic testing was not considered for the purposes of distinguishing between new and established office visits. Is this correct?

New vs Established

Did you bill for the reading of the Stress test? If so how was it submitted to the insurance company? I have billed for a doctor who would read patients results before he saw them in the office. However, we never billed for it as a separate charge. I'm confused as to how you submitted the charge.
