I billed the following:

06/12/14 99223 AI 25 06/13/14 99232 25
92928 LC 93306 26
92928 LM 59
93454 26 59

Medicare is denying the 99223 and 99232 stating Payment adjusted because the payer deems the information submitted does not support this many/frequency of services.

Is there anyway around this. He did the work (H&P), so why won't they pay. Thanks for any info.

if the 99232 was same day as heart cath you can not bill that even with 25 mod but if it was the day after it should pay-99223 with 25 mod same day as cath should pay
