Wiki New to ASC Billing - Help ASAP!

Can someone clarify how to bill for Surgical Room? Do we submit two claim forms, 1. HCFA-1500 for Procedures and 1. UB-04 for the use of the Surgical Room?

Second question: POS-24, does that need to appear anywhere on the UB-04 form? I do not see a box specifically for POS, however, my doctor seems to think it needs to be on the UB-04 form. Please

ASC UB vs 1500

1500 02 form is for physician billing - you can review NUCC for all instructions
UB-04 1450 is for facility billing - specific information belongs to each of the FL form locators (boxes)on the claim form UB04

After you read the instructions for both, you will understand the process. And when it comes to medical billing you always need to have all info in writing.
