In an hospital setting, in outpatient clinics located in the hospital, if a patient is seen in different specialty clinics, do i code it as a New pt every time pt is seen in a different specialty clinic or would that be considered as an Established pt as was seen within the hospital system regardless of the specialty.

For example pt seen in by Family care by a Internal medicine doctor and the next month seen by a another specialty doctor in Asthma clinic. Both these clinics are located in the same hospital settings and both claims are being billed under same Tax ID. Should i code the Asthma clinic visit as a New Pt or Established Pt. Because pt is seen for the first time in the asthma clinic but is a pt of Family care clinic already located in same settings.

If you are coding for the physicians, you will follow the CPT guidelines, and the patient will be new if they are new to that provider and specialty in last three years. Each specialty is treated separately, even if it’s in the same group or clinic.
