Wiki New pt consults for hx of cancer dx code

Greetings,
May I please get some assistance and feedback on the following question? When a new patient comes in to establish care with a Hem Onc provider for a history of cancer can we still code this with dx Z08 as the primary? The history of cancer code is an unacceptable primary code but it is not technically a follow up visit either. Thank you in advance.

Hi I would use the Z08 followed by the history code. If it is not a follow up related to her history of cancer, why is she seeing Hem Onc?

The pt is being seen for the hx of cancer only to start back up on being followed so I agree on using Z08. Thank you very much!
