A provider opened acnew own practice with new tax ID.

The provider was belonged to a large group practice, before opening own practice.

The many patients from large group practice will be coming to this new practice.

These patients' claim will be using NP ?? (these are old patients to this provider)

According to CPT, the first step in the new patient vs established patient decision tree is whether the patient has received any professional service from the physician or another physician in group of same specialty within the last 3 years. Since the patients have received professional services from that physician, I would say they should be established patients.
