Hello!
If a patient came in office in 6/2018 has an office visit that is send to the insurance, and has visits in 2020 that are selfpays for botox,when the patient comes in 12/2021,and the visit is send to insurance is the patient new or est?

The patient is established. Whether a patient is new or established is not dependent on the method of payment for services received.
