We are looking to re do our route sheet for the clinic. Does anyone have one that works good for your clinic. We would like one that the doc can tell us if the visit is global-new fx-followup join replacement- new pt etc. If you could fax it to me that would be great thanks cora 414-321-0632 please put it attention cora
