Wiki New DSM 5 vs ICD 10

I currently code for an inpatient Mental Health Facility and my problem is this. When coding from the DSM 5, some codes do not correlate with a the ICD 10 and several of my providers are not specific enough in their the dxs. My question is what is the rule with using Provisional vs History of in terms of an accurate dx?
