Wiki new cpt insurance submission requirements

We are using a new cpt code 76499-unlisted diagnostic radiographic procedure- tomo for breast.- is it a requirement to submit w/report with our claims?

I always submit the report when using an unlisted code as well as a letter describing the work involved and what procedure it may most be like and what reimbursement we expect. This usually saves a lot of time becaue they are probably going to ask for this anyway.
