The physician says the procedure performed was a block of the L5 dorsal rami branch nerve with additional levels, lateral branches of S1, S2 and S3 (all on the left side). I have one physician who want to bill 64450 X4 and another who wants to bill 64493 x1 and 64450 x3. I did a search and was not able to find a definitive answer. What is the correct way to bill this?
 
What is the purpose of the procedure> Is blocking L5 for treatment of the L5-S1 facet joint.

What is the ICD-9-CM being used for L5 procedure.

If the purpose and condition revolves around treatment of the SI joint. Need to confirm if L5, S1, S2, S3 is for innervation of the SI joint then this seems this would be 64450 x 4
