Can anyone tell me if a ballon angioplasty were done on the left subclavin vein and the innominate vein, would it be coded twice with the 35476 x 2. Or is it considered the same vein and only code it once?

I would say x1. I would be cautious to be x2 unless you can show subclavin was not primary procedure.
