Can anyone provide CMS article for the CPT 95867 and 95940 used in ENT procedures. And also please advise can we bill CPT 95867 and 95940 with 69635/ 69661 together. If we bill these codes together will we receive any separate payment for Medicare insurance or is it inclusive with ENT procedures?
