when an adolesscent comes in for an annual well child visit which ICD-9 code should be used V20.2 routine child or V21.2 Adolscent should we be using?

V20.2 until they turn 18, then V70.0. V21.1 is not for a routine exam, it's just an informational code stating that the patient is an adolescent. And no, you don't need to add it to the claim - V20.2 is all that's required.
