patient presenting to the office with morbid obesity and with a history of previous hiatal hernia repair with mesh and Nissen fundoplication. The patient has severe gastroesophageal reflux disease. She presents with gastroesophageal reflux disease and would like to undergo revision of her surgery to Roux-en-Y gastric bypass.

A little late, but I'm afraid you would've had to post the Op note to get a more precise answer. The lap Roux-en-Y gastric bypass is 43644, but the EGD is not a separately billable service. The redo on the hiatal hernia repair would depend on the documentation, it may or may not have been a separately billable service.
