My doctor has CPT 25118 X 2 COMPARTMENTS. His OP NOTE states he made 2 incisions on the left dorsal wrist and hand. The mass was found to encase the extensor tendons of the left hand involving the common extensors to all digits as well as partially involving the extensor pollicis longus tendon. The mass extended back through the dorsal retinaculum into the distal portion of the wrist. OP NOT STATES: Operation performed: Tenosynovectomy of the left wrist and hand. Can someone tell me if I can code this same CPT TWICE on two separate lines OR once with a modifier? Please Help.
