I'm confused on the 1997 guidelines for the HPI. It states you can use elements or chronic problems. If the patient comes in with ankle pain and he wants to go over his chronic problems. Can you use the elements and the chronic problems?
I'm so confused please help.

Either Elements OR STATUS of chronic problems

For the 1997 guidelines you audit the HPI based on EITHER the elements (timing, duration, location, etc) OR on reporting the status of 3 chronic conditions.

You cannot mix these.

So if your HPI reads like this:
Patient here for ankle pain. (location). His DM is well controlled with BS below 100 on all reads (Status of 1 chronic condition)

You would have only a brief HPI.
