procedure:rt sided L4-5 and L5,S1 laminectomy decompression foraminotomy
decompression of spinal nerve roots L4-5 and S1
a small incision was made in the lumbar spine followed by placement of the metrx tube. we then placed a size 8 metrx tube quadrant retractor over the metrx tube and it was attached to the bed in the usual standard fashion. it was expanded and the lamine of L4-5 region was seen in the operative field. a laminectomy decompression was under taken using a combination of midas rex and kerrison punches. the existing nerve roots of L4 and L5 on the right easily seen. some decompression was done to the left side of the patient to the L5,S1 region. a significant amount of arthrosis was seen, especially at the L5,S1 region. a ball-tipped probe was easily allowed to pass under the foremen. the nerve roots were easily traced. gross hemostasis was achieved. a small amount of steriod powder was placed onto the nerve roots. once this was complete,layed closure was performed.
