OK here is the seniaro.....

1st visit) Pt comes in for office visit complaining of upper resp symptoms. Nothing else is wrong. Review of systems good, healthy young patient or other problems, diagnosed with URI. Dr. states that he is ordering blood work since pt has positive family history of cardiovasular diesase. Office visit was charged.

2nd visit) Pt comes in for lab work. venipunture charged

3rd visit) Pt comes in for a follow up for lab work only. (Dr. wants pt to come in for those, not over the phone) all lab work was good nothing abnormal. Pt not have any symptoms from 1st visit. What would u code this office visit?

What about V67.59 and V17.49? I don't know what the note supports and I don't know if either of those will get paid but based on your description it sounds like that is about as good as it will get.

Hi,
Followup visit of patient require physical examination or objective examination.
If that documentation contains Subjective, objective with plan, V67.59 shall be reported for 'other followup examination'.
