Good morning everyone,
Do we give full credit for review of systems that state the following:
- Review of systems otherwise negative
- Review of systems otherwise negative, except stated above in the HPI
- Negative, except stated above in the HPI
- All negative
- Negative unless as mentioned in the HPI, all systems reiviewed all HPI
- All 14 review systems reviewed and found to be unremarkable

As an auditor, I will not give full credit for the ROS if the patient's presenting problems do not support it. For example, if a patient presents with a chief complaint of headache, and nothing more, why would the provider perform a full ROS?
Far as the statement" As stated in the HPI", you can pull the systems from the history for the ROS, HOWEVER, you cannot give double credit. You either credit the HPI, or the ROS. Not both.
