HELP!!!! I have a patient who had Excision of chronic left chest wound with fasciocutaneous flap closure. I billed 15734, insurance denied the claim stating Op Note did not support billing code. Can anyone give insight on coding for me??
This is the diction from the op note:
Attention turned to larger inferior wound. This was similarly excised. Sent for culture and pathology. Patient noted to have large mucosalized cavity extending essentially to the midline in the suprafacial plane. This was rongeured to remove pseudomucosa. Intercostal neurovascular bundle indentified and dissected free. Atricure cryoablation device then used for 2 minute treatment.Inferiorly fasciocutaneous tissue from the external obliques elevated and rotated into resulting defect. 10 flat jp drain placed and secured. Wound copiously irrigated. Hemostasis obtained with bovie. Closure completed with fascial 2-0 monocryl, deep dermal 3-0 monocryl and nylon at the skin. Dressed with xeroform and gauze.
