patient came in thru er and dr did card cath / stemi and stent and i coded as 93458/92941 (for Bare metal stent- rca) and within a week pt came in again in er and dr perform as emergent card cath and did balloon angio and remove thrombosis around the stent placed week before. my question is should i only bill for balloon angio or card cath also. i am hospital cath lab coder.

Percutaneous Coronary Interventions incl catherization

according to the section guidelines for PCI, selectively catheterizing the vessel is included in the service and need not be separately assigned.
 
The cath for the second admission is separately reportable if the patient's condition changed. Why did they come to the ER again? Chest pain, possible STEMI? Either way it sounds like a change in their condition and should be billed.
