Wiki Need dx for cancellation of surgery

Physician cancelled colonoscopy due to lack of prep by the patient. I know I will use the V76.51 and V64.1 but does anyone have any idea how to code lack of prep?

Poor prep I always used the V64.3 not the V64.1
