Wiki Need assistance with E/M Bullets

Hi there. I would like to ask assistance. We are trying to make a new template for our physician and I would like to know where I can get a listing of what exact bullets have to be met for each level of OV....like 99213, 214, 215 and np codes 99203 99204 99205...Any help would be appreciated. :)

Your best bet would be to use a 1997 E/M audit sheet as your guide.
