NC Medicaid has been denying 96415, 96366, 96367, 96375 as bundled (POS 11) but not consistently - some claims do get paid. Some Medicaid reps advise over the phone to append the 59 modifier. Nothing in writing, and no references are made to any issue of the NC tracks that would specifically address the add on codes and the sequential codes. If anyone has a link (links) to something in writing from the NC Medicaid, I would appreciate if you share.
