The weight only applies to intramural fibroids (ones that are imbedded in the uterine wall), not surface myomas or a combination thereof. So if all of myomas were intramural with a weight over 250 g you go with 58146. If they are all surface myomas you code 58140.

Hi! Ive been following your guidance on things for quite a while! You seem to be the most knowledgeable! I have a patient who had an elective abortion. We did not not perform the elective abortion but she has come to us for a followup visit and our GYN Doc performed a pelvic exam only. Found no complications. And referred her to her PCP for labs. We did no ultrasound either. Im at a loss as what code would be appropriate. Ive look at alot of codes but cannot find anything that is pointing to us as doing the actual abortion. Its just a follow up as a NP with pelvic exam. Its just the reason she wanted it. I was going to use the Z09 but am unsure if that will pay. Any assistance would be great! Thank you so so much, Teresa

Teresa, in future would you please start a new thread with appropriate title so the information we share on this topic can be quickly accessed by others. In this case Z09 would in fact be the reason for the visit as this a follow-up to a completed treatment. But I wonder why the patient was not seen by the group that did the abortion for this follow-up? And I assume you are not in any way affiliated with the provider group who did the abortion service so that follow-up is not included.
