Physician states in his op-note "surgical repair of 4 separate ventral hernia, all different locations, different sites; primary repair." He goes on to describe the location of each hernia and how he repaired each of them. The MUE for 49560 is 2 however the insurance is only allowing for 1, is mod. 59 acceptable because each ventral hernia was separate and repaired individually? Thank you in advance for the help!
