Wiki Multiple procedures

Please help, my physicians gave me a procedure to code out and I'm stuck as this is unfamiliar to me

His procedure note states the procedures as follows:
1.Provacative lumbar diskography L3-4, L4-5, L5-S1 under fluoroscopy
2. Lumbar diskogram
3. Fluoroscopic guidance
4. Left selective nerve root block under fluoroscopy L3, L4, and L5

I am so confused, can any one help me out with this one. I know there are two separate procedures which I will need to apply the 59 to the second procedure. when I put it into the system it looks crazy

Im sure for the first procedure I should use the dx code 722.10 62290 and for the additional code 72295 and for the SNRB use the dx code 722.52 and 724.02

PLEASE HELP

I have been holding this report for over a month and I cannot figure out how to code it and I have no other coder in the office to ask.

Code 64483 is a component of Column 1 code 62290 and cannot be billed using any modifier.


Above is from CCI check. I would go back to the surgery scheduler and the precert dept and physician and state, these two procedures can not be performed together.

You will only be able to report the discogram 62290 x 3 72295 x 3
