One of our Surgeons have been doing multiple level lumbar endoscopic radiofrequency injections. The operative report is stating "Lumbar Endoscopic Radiofrequency Ablation, Left L2-L3, L3-L4, L4-L5, L5-S1, Left L2-L5 Lumbar Medial Branch Block" The Coding company we use is sending the coding back showing 64999 only one time. Is this correct? I know the 64999 is the correct procedure code but should there only be one procedure code and one line item or should it show multiple units? PLEASE HELP
