How would I properly code this?

Would I use 280.9 AND 285.21 AND 280.0?
Or just 285.21 and 280.0? Or just 1 code?

My thought is... 285.21/585.X and 280.0/578.9. I can't find one code to fully describe these conditions. If you find out, let us know.
