Pre op DX:
Failed Multiple anterior cruciate ligament reconstructions of Left knee, painful hardware Left knee.

Post op DX:
Failed ACL rev
Painful Hardware
Torn medial Meniscus
grade 3 patellofemoral chondromalacia
grade 3 medial compartment chondromalacia
grade 2 lateral compartment chondromalacia

Operative Procedures:
Arthroscopy, arthroscopic partial meniscectomy, hardware remova, bone grafting of tunnel defects, femur and tibia.

Procedure (condensed)
Standard portals made. Inspection of suprapatellar pouch showed moderate synovitis. Articular surface of patella and trochlea had grade 3 changes present. These areas were debrided with the shaver. Medial and Lateral gutters were clear. Medial compartment had a tear of the posterior horn of medial meniscis. It had a partial medial menisectomy with secondary tear in the remaining meniscus. Once this was resected, the articular surface was evaluated showing extensive grade 3 changes of the medial femoral condyle and the tibial plateau. These areas were also debrided. The ACL was absent. The PCL was intact. In the lateral compartment, there were grade 2 changes of the articular surfaces with evidence of a prior partial lateral menisectomy.

Attention directed anteriorly. An incision was made in the old scar. A screw in the tibia was identified and removed. Incision was made removing the old scar superior and laterally. The fascia was divided. The two staples were initially identified and removed. A Transfix pin was then removed. The interference screw was removed from the femoral tunnel. With the arthroscope in place the tibial tunnel was identified. It was drilled to remove the remaining graft within the tunnel. A combination of bone putty plus croutons were used to fill this tunnel. The femoral tunnel was also drilled and was filled with putty using a cannula to guide the insertion of the putty. Due to the lateral incision, addtional bone graft was made in the areas where the hardware was removed.
