Wiki More diagnosis codes than allowed as diagnosis pointers on CMS 1500 claims

CMS 1500 claim forms only allow (4) diagnosis pointers per line item. What do you do when you have more than (4) diagnosis codes and they all apply? Will Medicare allow billing bilateral on separate lines in cases such as this?
