Pre-procedure diagnosis:
molar pregnancy
Post-procedure diagnosis:
molar pregnancy
Procedures performed:
suction and sharp curettage
Technique/Procedure:
Patient was taken to the operating room placed on the operating table. General anesthesia was induced. Patient was placed in the lithotomy position in yellowfin stirrups. The vagina and perineum are prepped in usual sterile fashion a Foley catheter was placed to continuous drainage and the patient was draped in the usual fashion. Weighted speculum was placed in the vagina the anterior lip the cervix was grasped with a ring forceps. 12 mm suction curette was placed and rotated in a clockwise fashion as it was withdrawn with retrieval of abundant products conception. This process was repeated multiple times until there was no further tissue obtained. Sharp curette was then used and then was noted to be further products conception circumferentially. The suction curette was then placed and further products conception was retrieved. The uterus was now down to about 12 weeks size from 22 weeks size at the beginning of the procedure. Patient was given 10 units of Pitocin IM. Ultrasound guidance was requested and the ultrasound tech presented to the operating room. There appeared to be a small fragment of tissue posterolaterally on the patient's right. This tissue was retrieved with a sharp curette. The endometrial lining and appeared normal. The suction curette was passed 1 additional time to assure that there was no retained fragments of tissue. The procedure was then terminated. There was good hemostasis. Patient was given an amp of Hemabate as a precaution. She is taken to the recovery room awake extubated and in stable condition she will be ready covered there and then transferred to the GYN unit when adequate recovery has been completed.
