When you are submitting the notes and the pathology, are you also submitting a MOHs map/form? We also very frequently get denials on our records stating that the records do not sufficiently support the doctor acting as both surgeon and pathologist. The MOHs map sheet we use contain all of the specific MOHs info, including time in/out for each stage, the drawings of the site(s), why the MOHs was being done in the first place (suspected or proven and what Dx), as well as the traditional pathology notes all in the provider's hand (like inflammation present, margins clear, etc, etc). For some reason this seems to get us paid, even though it contains *exactly* the same information as the chart note.
