Wiki Modifiers X1-X4

Is anyone else receiving denials for not adding the X1-X4 modifier from Medicare? The outpatient facility I work for has stated that we need to add the X1 modifier to all claims but, I do not agree. Most wound care patient receive care and heal, that modifier does not suit wound care situations. Can anyone assist?
