We had a patient that was in rehab after surgery and rehab wanted her to see her pcp to be released. Her insurance has denied since she was in a facility. Is there a modifier that can be used to get the E/M paid?

Id bill with a facility E&M code and not office

If the payient was seen in the office setting, you use the visit level with the favility POS. If your provider performed the visit in the favility then use facility visit levels with the favility POS.
