I work for an FQHC in a very seasonal area, with a huge influx of summer visitors. We are a primary care clinic, not urgent care, but we see people on an urgent or walk in basis as the nearest urgent care is over 30 minutes away and the nearest ER over an hour (on a good traffic day). For patients who have HMO plans, we ask them to sign a referral waiver and get a referral. We request the referral to the provider they will be seeing on that date of service. Often times if they need to come back, they will see a different provider.
My question is: If we have a referral to Dr. Smith for 3 visits, the patient sees Dr. Smith on the first visit, but then sees Dr. Jones on the second visit, can we submit the Dr. Jones visit under the Dr. Smith referral with the modifier Q5?
