Does anyone know if there is a modifier that can be used when billing J0490 with 2 different NDC #'s such as J0490.

Are you using two numbers because you didn't have enough in one vial and the new vial has a different number?

It's usually to complete the dosage and usually there is not any discarded meds. If some of the meds are discarded, would the "JW" modifier be appropriate to use in conjunction with the "KP, KQ" modifiers?
