If the resident has a note and states at the bottom that he saw the patient with "Dr. So and So" BUT the physician did not do an addendum or his own note can you still code for that visit with modifier GC?
 
Sorry but the answer is no. It must be a first person notation from the Preceptor. This can be found in the CMS Teaching Physician Guidelines. You can google "CMS Teaching Physician Guidelines" and it will guide you to a pdf that has the information for you.
