Wiki Modifier 59 - colonoscopy

During a colonoscopy the dr removes a polyp 45380 and stops bleeding 45382. The bleeding was not caused by the dr. Which cpt code would take the modifier 59. Thanks for your help

The 45382 would get the -59 modifier per CCI
