Wiki Modifier 58?

Does anyone know what the correct modifier is when a Medicare patient had closed treatment for a distal radius fracture and a couple months later developes post traumatic carpal tunnel syndrome? Patient returned during global period for a Carpal Tunnel Release.

I would use modifier 78 in this case, under the assumption that the new condition is related to the original injury.

Modifier 58 would not be correct as this doesn't meet the definition of a 'staged' procedure, which is either planned in advance at the time of the original surgery, or a second and more extensive operation to treat the original problem.

I definitely agree with modifier 78. I'd also just like to add:

Modifier 58: Extension of disease/disease progression/more extensive procedure. Knew you were going to do the surgery.

Modifier 78: Complication of original surgery (ex: clot, infection, bleeding). Has to be done in OR.
