THE DOCTOR SEES THE PT FOR OBESITY AND DECIDES TO GIVE HIM B-12 INJECTION. CAN WE USE MODIFIER 25 TO E/M LEVEL? PT HAS PPO INS.

Yes, you will need the modifier 25. Also don't forget the appropriate ICD-9 code for the B-12 injection.
