Pt returns for f/u of rt shoulder pain. Dr examined pt 5 mos ago and he injected the subacromial space. Dr did a quick exam and did xrays. Decision was made to reinject the subacromial space. Dr put a modifier 25 on the E/M. Now....can I use the mod 25 since he injected him for the same shoulder 5 mos. ago?

If this is for the same problem I usually bill just the injection code if it is less than 6 months since he had the first injection and not bill the office visit.
Otherwise, yes, you put a 25 mod on the E/M if billed with a procedure code that carries a zero or ten day global period.
