If a patient came in the ER to have sutures removed and had the cut evaluated would you append a modifier 25 to the EM level

No. But was it infected and wound cleaned...if this is case you d add new dx and CPT define procedure of debridement or bx, Etc. But just review healing of wound not add modifier 25. Now if pt came in and got injection after review for some reason/complication started..yes add modifier 25.

What else are you billing that -25 on the E&M to indicate a significant and separately identifiable visit is a consideration?
