Wiki modifier 25 - gynecological exam

When billing an annual gynecological exam with a problem visit I always put the modifier 25 on the a/e not the e/m. I use it on the e/m when billing with a procedure. Is this correct?

25 modifier goes on the E/M, not annual exam.
