Wiki Mod 25 on E/M with 925** codes?

Hi fellow coders,
Should we be putting a modifier 25 on E/M codes 992** when billed with 925** codes. They have no global days but it was suggested to me that we should have a modifier 25.

The audiologist performs the testing and the physician reports the results to the patient and documents for E/M billing.

The audiologist is contracted through the physician's office and does not do their own billing.

Hi Teresa, we have never appended -25 to the E/M code when the results are given the same day as any audio testing. none of our payors here in Florida require it but you may want to check with yours.

I have NEVER added a -25 to the E/M with the audio testing. However, I did get a rejection on the E/M from BCN today, saying the E/M was inclusive. A call was put in, but no answers yet.

What about 92504, do you append a modifier 25 to the E/M? NCCI edits do not show a conflict, but some coding articles I have viewed suggested we use the modifier due to the "separate procedure" status of the CPT.
