I have a question on compounded drugs.
If you purchase the drug from a company (Avella) that pre-mixes the drugs, is that drug then considered a compounded drug on your end for coding and billing purposes? Or would I just consider this drug mytomycin and use that NDC#
The drug has mitomycin, mannitol and sterile water per our invoice. Unfortunately, the NDC# given in our nursing documentation has a letter in it. For what I have seen over the years NDC# do not have letters, correct?
If I would code out as a compounded drug do I use J3490 for all the HCPCS and then their own individual NDC with qty and KP/KQ modifiers?
