Are medication refills included in annual gynecological preventive exam? Is there a list of what is included in that exam and what would be an additional office visit charge?

Not unless the documentation clearly supports significant additional work related to the refills. For instance, I go for my annual and my BP is normal and I am on BP medication - my provider orders my yearly renewal for BP meds - in that case included. On the other hand, I have an out of control BP reading (two of them) and have gained weight and the provider wants to increase my BP medication level- that might constitute a separately billable E/M service if the evaluation is adequately described.
