I work for an ASC in NJ and was wondering if anyone else is getting denials from Medicare Part B for denial code CO-151? "Payment adjusted because payer deems the information submitted does not support this many/frequency of services". Does anyone know what this means? We are getting them for all different procedures - i.e., excisions/closures, colonoscopies, EGD's, etc.
